Eggshell Therapy, Inc. is a private counseling practice and not a managed care driven mental health clinic. I work for you not the insurance company. If you choose to use insurance for counseling reimbursement, I can work with you as an out of network provider. Usually PPO or POS plans are in this category.

If you have a PPO plan, it is likely that you can see me as an out-of-network provider and receive partial reimbursement from your insurance company. If you would like to access this benefit, you may choose to call your insurance company directly and begin by inquiring the following:

  • How much does my plan cover for an out-of-network provider?
  • What is my out-of network deductible and has it been met?
  • Do I have to have a “parity” diagnosis (more severe) to qualify for out-of-network benefits?
  • How many sessions per year does my plan cover?

The amount your insurance reimburses you is typically 50-80% of the allowable rate, based on your plan, once you have received any prior authorizations, and your out-of-network deductible is reached.

I can provide you with a receipt with the necessary info to obtain counseling fee reimbursement based on your plan.
Specific rate information will be discussed in person or over the phone.

You may pay with cash, check, or a credit card on the day of services are rendered. If you pay with your credit card, the name Eggshell Therapy, Inc. will appear on your credit card statement.

When a client cancels a session, there is a shifting in our specific agendas to accomplish our mutually agreed upon goals that often required prep time and significant planning on the part of the Therapist. The best chance of recovery possible requires you to make treatment a priority in your life. This requires steady consistent practice of skills necessary for a successful treatment outcome. Therefore, we request 24 hours notice of cancellations. Session cancelled within 24 hours will be charged a fee for the session. Exceptions to this would be for cancellations that involve emergencies, such as death, illness or illness of a child. If the Therapist, for some reason, is running late -- that time will be made up at the end of the session whenever possible or that time will be added to the next session. If we are able to offer your cancelled session time to another patient, you will not be charged for that time.

In- Home therapy sessions are billed at the same rate as treatment that is provided in the office. Sessions are prorated to determine your fee. Time spent traveling to your home or to off-site locations for therapy, consultations, or time spent in after hours phone calls will be billed in the same prorated manner.

Many protocols may require out of pocket expenses to our clients. Items purchased on behalf of your treatment are yours to keep and are not our property. Generally, these total less than $100 but may include such items as: Treatment Workbooks, journals, DVDs, CDs, recording devices and books. We try to keep any required items in stock for our clients, however you may purchase any items outside at places such as and other online or offline sellers.

I am not a covered provider for Medicaid, Medicare, or any State / National aid. In addition, I do not accept Workers Compensation at this time.
